Who to send

Patients going home who need primary care that comes to them: a visit in the home instead of a trip to the clinic. We see the patient where they live, whether that is a house, an apartment, assisted living or a family member's home.

What we do

Primary care in the home, including ongoing management of chronic conditions, medication review, and the orders and paperwork that go with home health. We order mobile diagnostics such as labs, x-ray, ultrasound and EKG and review the results; we do not perform them ourselves. Electronic visits are by video call.
