Refer a patient

For discharge planners, case managers, assisted living staff and home health teams placing a patient with primary care at home.

We take over primary care for patients who are going home and will struggle to get to a clinic. Chronic disease management, medications, and the orders that keep them out of a waiting room.

Visits are by Deborah McKnight, PhD, ANP-C, who owns the practice, with Dr. Juan Zamora, MD as collaborating physician. Both are on Medicare Care Compare.

We are accepting new patients. If a patient is going home and needs primary care that comes to them, send them.

Questions we get from referrers

Before you send one

Does the patient have to be homebound?

No. Homebound is a Medicare rule for home health agencies, not for a provider visiting the home. If getting to a clinic is hard on the patient, that is reason enough.

Can we refer if the patient already has a primary care physician?

Yes. Many patients transfer because their existing physician has no way to see them at home. The patient chooses.

Do you work with our home health agency?

Yes, that is the normal case. We coordinate with the agency's nurses rather than around them, and agencies have their own portal.

What if the patient is outside your usual area?

Call and ask. We occasionally travel further for established patients, and if we are not the right fit we will say so rather than leave you waiting.

Have a patient to place?

Call or text and describe the situation. We will tell you plainly whether a house call fits, and what the first visit looks like.
