Homebound is a Medicare rule for home health agencies, not for a provider coming to the house. They do not have to be homebound to be seen at home.

A nurse practitioner examines the patient, goes through their conditions and medications, answers your questions, and writes prescriptions or orders tests as needed.

You can be there, or on the phone. Nothing about the visit requires you to take the day off.

You do not need to be their legal decision maker to call and ask questions. To send a referral you will need their permission, or their legal representative's, to share their information, and the form asks you to confirm that. If you would rather not phone, the referral portal takes referrals from families, patients and healthcare professionals alike, and asks who you are as you go.

If you have paperwork from a hospital or a home health agency, you can attach it in the portal as a PDF or a photo from your phone. You do not need to have it to get started.

On Original Medicare (Part B), no. On a Medicare Advantage or commercial plan, yes: call the number on the back of their card and ask the plan to list Deborah S. McKnight, ANP-C, NPI 1528005758 as their primary care provider. We need this done before the first visit.

Call and we will tell you the soonest we can get to them. It depends on where they live and what is going on with them, so we would rather give you a straight answer on the phone than a promise on a web page.

Examination, ongoing management of chronic conditions, medication review and prescriptions.

We also order mobile blood work, X-ray, ultrasound and EKG that come to the house, then review the results and decide what they mean for their care. Electronic visits are available when a question does not need someone at the door.

We are not an emergency service. If something is an emergency, call 911.

We do not perform the imaging and lab work ourselves. We order it and review it. We are primary care, so specialist care and hospital procedures stay with those providers.

Adults of any age. Deborah is a board-certified adult nurse practitioner, so adults are what the practice is licensed and equipped for. We do not see children.

Most of our patients are older, but the reason we come to the home is that leaving it is hard, and that is not only true of seniors. If your adult son or daughter needs a provider who comes to them, call and describe the situation.
